Doctor Referral

A doctor referral is not needed to see a physical therapist. However, some insurance companies may require them when submitting your receipts for reimbursement.

Home Visits

Home visits are NOT offered by the clinicians at this clinic

First Appointment

Have your intake form completed prior to the start of your appointment. Bring any diagnostic imaging, lab results, surgical reports, or referrals with you. Wear loose fitting clothing and bring indoor gym shoes if applicable.

Treatments

Treatments will comprise of manual therapy and guided therapeutic exercises. Modalities such as TENS, IFC, and ultrasound are not offered at this clinic as research indicates they have poor therapeutic benefits.

Rates

Initial assessments are $150 and follow up treatments are $150 for 60 mins or $85 for 30 mins.
